Patients who live in the following area are welcome to apply to register with the Practice. You may see the Doctor of your choice if they are available, but you should be prepared to see any Doctor in an Emergency.
The Practice Area is South of Weyhill Road, to Weyhill, Monxton, Abbotts Ann, Goodworth Clatford, Wherwell, up to the A303, to A3093 Churchill Way, along Northern Avenue, Western Avenue, Western Road back to Weyhill Road.
Follow the signs for the Iron Age Museum from anywhere in Andover, you will find us in the same road, Church Close, on the left before you arrive at the museum. We provide access and washroom facilities for Disabled Patients
Pre-Registration with the practice
If you wish to pre-register click on the link below to open the form. When you have completed all of the details on both pages, click on the "Send" button to mail your form to us. You will still need to visit the surgery to sign the form to confirm that the details are correct and to provide proof of identity.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct. We are unable to register you fully unless we have all of the relevant information and that the online forms have been completed correctly.
